Folded cotton underwear on neutral linen.

The short answer

Buy a mixed pack with both rises first, then buy more of whichever you end up reaching for. Over-bump gives support and a smooth line and runs warm; under-bump is cooler and slides down once the bump is heavy. Nobody can tell you which you will prefer, and a mixed pack settles it for the price of guessing wrong once.

Over-bump or under-bump

Over-bump

A full panel coming up over the bump. More support, smoother line under clothes, and nothing to slide. The complaints are heat — it is another layer across your abdomen all day — and the top edge rolling once the bump pushes against it.

It is also the rise that matters later: after a caesarean, a waistband sitting on the incision is painful, and a high rise clears it entirely.

Under-bump

A wide band sitting beneath the bump. Cooler, invisible under fitted clothes, and preferred by a lot of people through summer. Its failure mode is gravity: once the bump has real weight, the band works its way down over a few hours.

Foldable

The underrated third option. A fold-over waistband means the same pair is over-bump when you want support and under-bump when you want to be cool, which is genuinely useful across a pregnancy that spans two seasons.

Fabric: why cotton is worth seeking out

Stretched skin itches. It is one of the more commonly reported discomforts as the bump grows, and ACOG's material on skin conditions in pregnancy covers the broader territory. Synthetic fabric against itchy, stretched skin makes it worse; cotton does not.

Add to that the fact that pregnancy makes you run warmer and sweat more, and cotton or a cotton-rich blend becomes a sensible default rather than a preference. The trade is that cotton loses shape through a day and needs washing to recover.

Seamless synthetic has its place, which is under fitted clothes where a cotton waistband would print through.

How many do you need?

More than for ordinary life, for three reasons: you will be washing more often, you may have more discharge (which is normal in pregnancy), and later on bending to sort laundry is less appealing.

Eight to ten pairs is a comfortable number. A mixed pack of six plus a couple of better-made pairs covers it, and the better-made ones are what you will still be wearing afterwards.

Do not buy all of them at once in one rise. Buy a mixed pack, find out which you prefer, then top up.

The postpartum overlap

Worth planning for, because it changes what to buy now.

For the first week or two after birth you will want mesh or disposable pants rather than anything you own — they hold a maternity pad, they breathe, and you will not mind ruining them. That is a separate purchase and it is covered in postpartum underwear.

From week two onward, high-waisted maternity underwear is what most people move into, and it is the thing that keeps getting worn for months. That is the argument for buying a couple of good high-rise pairs now rather than only cheap packs: those are the ones with a second life.

How we picked

Six axes: whether it stays put, fabric comfort, breathability, size range, whether it survives washing, and cost per pair. Staying put and fabric comfort are weighted hardest, because those are what the reported complaints are about — sliding waistbands and synthetic fabric on itchy skin.

We read the current listings and the reported experience; we have not worn these. The method is on how we pick.

One thing worth mentioning to your provider

Increased discharge is normal in pregnancy. Discharge that is itchy, sore, smells unusual, or is green, gray or bloody is worth mentioning to your midwife or GP rather than assuming it is ordinary. So is any fluid that might be leaking rather than discharge, particularly before 37 weeks, which needs contacting your maternity unit now.

8.8/10

Kindred Bravely High Waisted Postpartum and Maternity Underwear

The best-made option here and the only one you will still be reaching for in six months. A high soft waistband that works over the bump now and clears a caesarean scar later, in fabric that is pleasant rather than merely functional.

Pick 2 · Over-bump support that does not dig in

8.6/10

Momcozy Maternity Underwear High-Waist Seamless Over-Bump Briefs

The most engineered pair here: an expandable belly panel with breathable mesh, which addresses the two complaints about over-bump underwear at once — the waistband digging in, and the heat. Seamless, so nothing shows.

Rise
High, over-bump

What works

  • Expandable panel rather than a fixed stretched waistband
  • Breathable mesh panels
  • Seamless under fitted clothes
  • Real belly support rather than just coverage

What it gives up

  • Over-bump styles still run warm
  • Mid-to-high price
  • Panel visible under thin fabric

Pick 3 · Cotton, in quantity

8.4/10

Angelhood Cotton Maternity Underwear Under Bump 8 Pack

Eight pairs of cotton under-bump briefs, which is the right combination if you want breathable fabric and enough pairs that you are not washing constantly. Cotton matters more in pregnancy than people expect, because stretched skin is often itchy.

What works

  • Cotton, which stretched itchy skin prefers to synthetic
  • Eight pairs — enough that laundry is not a daily event
  • Comes with a laundry bag
  • Works through pregnancy and postpartum

What it gives up

  • Under-bump waistband slides down once the bump is heavy
  • Cotton loses shape through a day
  • Plain styling
  • Sizing runs variable

Skip it ifyou are late in pregnancy — under-bump styles slide by then.

8.2/10

Amazon Essentials Maternity Underwear Under Bump and Over Bump 6 Pack

The practical way to settle the over-bump versus under-bump question: a pack with both, at the lowest cost per pair here. You will know within a week which camp you are in, and the other half of the pack still gets worn.

Pick 5 · Wearing over or under, as the day goes

8.1/10

Intimate Portal Maternity Underwear Cotton Foldable Under the Bump Briefs

A foldable waistband is a genuinely good idea: wear it up over the bump when you want support, fold it down when you want to be cooler. The same pair works in both configurations, which no fixed-rise brief does.

Pick 6 · Stopping thigh chafe in warm weather

7.9/10

INNERSY Maternity Underwear Under Bump Boyshort 3-Pack

A specific problem, solved: thighs that are closer together than they used to be, in warm weather, chafe. A boyshort cut covers the inner thigh where a brief does not, and that is worth knowing about before a summer.

Questions people actually ask

Is over-bump or under-bump maternity underwear better?

Over-bump gives support and a smooth line and runs warmer; under-bump is cooler and invisible but slides down once the bump is heavy. There is no universal answer.

Buy a mixed pack with both rises first and then buy more of whichever you reach for. That settles it for the price of guessing wrong once.

How many pairs of maternity underwear do I need?

Eight to ten is comfortable. You will be washing more often, discharge increases in pregnancy, and sorting laundry becomes less appealing later.

Do not buy all of them in one rise at once. A mixed pack plus a couple of better high-rise pairs covers it.

Should maternity underwear be cotton?

Cotton or cotton-rich is a sensible default, because stretched skin often itches and synthetic fabric against it makes that worse. Pregnancy also makes you warmer and sweat more.

Seamless synthetic still has a place under fitted clothes, where a cotton waistband prints through.

Can I just wear my normal underwear?

Early on, yes, and plenty of people do throughout by wearing their usual briefs under the bump. The waistband digging in is the usual reason people switch.

What maternity underwear adds is a rise designed to sit over or clearly under the bump, and a waistband wide enough not to cut in.

Will maternity underwear work after the birth?

High-waisted maternity pairs are what most people move into from week two onward, and they keep getting worn for months. A high rise also clears a c-section scar, which matters.

For the first week or two you will want mesh or disposable pants instead, which hold a maternity pad and which you will not mind ruining.
