A person stretching their hip while sitting on the edge of a bed in morning light.

The short answer

Buy firmness, not size. Hip pain from side-sleeping happens when the gap between your knees collapses. Bead fill and memory foam hold that gap all night; polyester fiber fill flattens under the weight of a leg within a couple of hours. A huge soft pillow will disappoint you and a small firm one will not.

What is actually hurting

The pain is usually in the outside of the hip you are lying on, or deep in the joint, and it is worst in the early hours. Two things are producing it.

Direct loading. Your whole upper-body weight plus the bump is resting on one hip for hours on a surface that is not that soft. Pregnancy also relaxes the ligaments around the pelvis, so the joint is less tolerant of that than it used to be.

Rotation. With your knees stacked, the top leg falls forward, rotating the pelvis and twisting the lumbar spine. That asymmetry concentrates load on the hip rather than spreading it. MedlinePlus lists hip and back aches among the ordinary discomforts of pregnancy and points to support and positioning as the practical responses.

A firm knee gap fixes the rotation. A wedge under the bump reduces the direct loading. Doing both is the complete answer, and it is why we recommend thinking in terms of a two-piece setup rather than one purchase.

Why fill type decides this page

The ranking above is driven more by material than by shape, which is unusual for this category and is the specific insight of this page.

  • Bead fill — resists compression, adjustable, holds a gap all night. Most expensive.
  • Memory foam — holds height reliably, no setup, runs warm.
  • Polyester fiber — light, cheap, and the material that flattens. Fine for belly support, poor for a knee gap.

If you are choosing between two pillows of the same shape, the one with firmer fill is the better buy for hip pain, even if it is the smaller one.

How we picked

Six axes, with hip and knee alignment and “keeps shape overnight” weighted hardest for this page — a pillow that scores well on everything else and compresses by 2am has failed at the only job you bought it for. We read the current listings, the published specifications and the recurring complaints, which in this category are overwhelmingly about flattening and heat. We have not slept on these; the method is on how we pick.

Things that help that are not pillows

Worth saying, because the honest version of this page cannot be only a shopping list.

  • A mattress topper can matter more than a pillow if your bed is firm. The hip needs somewhere to sink slightly.
  • Turning over with your knees together rather than leading with one leg reduces the twist through the pelvis. This is standard pelvic girdle pain advice and it costs nothing.
  • Not standing on one leg to dress, and sitting down to put on trousers, reduces daytime loading that shows up as night pain.
  • A physiotherapy referral if the pain is limiting what you can do. Ask earlier than feels reasonable, because waiting lists are long.

When it is not ordinary hip ache

If the pain is over the pubic bone, in the dimples above your buttocks, or hurts sharply on single-leg movements like stairs and getting out of a car, that pattern suggests pelvic girdle pain rather than side-sleeping ache. The RCOG has patient information on it, the management is different, and a support belt may help during the day where a pillow will not. See our SPD guide and the pelvic-pain belt picks.

Pain with a fever, bleeding, regular tightening before 37 weeks, or reduced movements is a reason to contact your provider now rather than buy anything.

Every pick, in detail

Scores below are ours. They come from the published specification sheets and from the patterns in what buyers consistently report — not from wearing these ourselves.

Top pick · A knee gap that holds all night

9.0/10

bbhugme Pregnancy Pillow with Adjustable Firmness

The only pick here where you control firmness directly, which matters because hip pain is caused by a gap collapsing rather than by a gap being absent. Bead fill does not compress the way fiber does, and you can add fill as your bump gets heavier. Expensive, and the correct answer if the hip is genuinely the problem.

Shape
Two-piece, shapeable
Firmness
Adjustable — bead fill you add or remove
Bed footprint
Small, moves with you
Reuse
Converts to a nursing support

What works

  • Firmness is adjustable, so the gap holds in month eight as well as month five
  • Bead fill resists compression under the weight of a leg
  • Shapes to your actual hip width rather than a generic one
  • Converts into a feeding support later

What it gives up

  • Highest price here by a clear margin
  • Setup takes a few nights to get right
  • Faint bead noise when you move
  • No head support — you still want your usual pillow

Skip it ifyou need something cheap tonight, or you want a single out-of-the-bag object.

Pick 2 · Firmness without an adjustment learning curve

8.8/10

DONAMA Memory Foam Cooling Maternity Pillow

Memory foam is the material answer to the compression problem: it holds its height under a leg in a way fiber fill does not, and it needs no setup. The cooling cover is a sensible counterweight, because foam runs warmer than fiber. If you want firm and you want it to work out of the bag, this is the pick.

Fill
Memory foam, not fiber
Cover
Cooling finish
Firmness
Holds shape under weight
Bed footprint
Moderate

What works

  • Holds height under weight — the specific property hip pain needs
  • No adjustment or setup
  • Cooling cover offsets foam's main downside
  • Keeps its shape across months rather than flattening

What it gives up

  • Warmer than fiber fill even with a cooling cover
  • Heavier to move around the bed
  • Foam has a break-in period and an initial smell
  • Less wrap than a full U

Skip it ifyou sleep very hot — foam plus a bump is a lot of heat in one place.

Pick 3 · Hip pain on both sides

8.7/10

Momcozy U Shaped Full Body Maternity Pillow

If both hips hurt, which is common by the third trimester, the U earns its footprint: the arms sit between your knees from either direction, so you get the gap on whichever side you land on without moving anything. Fiber fill is the weak point for this specific problem, so check for flattening complaints before committing.

Shape
U, wraps both sides
Support
Head, belly, back, knees
Cover
Removable, machine washable
Bed footprint
Wide — plan for a queen or larger

What works

  • Knee gap available from both sides without repositioning
  • Belly supported at the same time, which reduces pelvic drag
  • No setup, no learning curve
  • Washable cover

What it gives up

  • Fiber fill is the material least suited to holding a knee gap
  • Half a queen bed
  • Traps heat on both sides
  • Heavy

Skip it ifonly one hip hurts and your bed is small — a J or a firm knee cushion is better value.

Pick 4 · Putting support exactly where it hurts

8.5/10

Momcozy Original F Shaped Pregnancy Pillow with Adjustable Wedge

The detachable wedge is genuinely useful for hip pain because the right place for support changes: some nights the hip wants something under it, some nights it wants the belly lifted off it. A fixed shape cannot do both. Less firm than foam or beads, but far more adaptable than either.

Shape
F, one-sided with detachable wedge
Adjustable
Wedge repositions for belly or back
Bed footprint
Narrower than a U
Cover
Removable, machine washable

What works

  • Wedge repositions to the hip, the belly or behind the back
  • Half the footprint of a U
  • Easier to get in and out of bed past
  • Wedge keeps working on its own after you stop using the body section

What it gives up

  • Fiber fill compresses more than foam or beads
  • One side at a time
  • Pieces to keep track of
  • The adaptability only pays off if you actually adjust it

Skip it ifyou want to set it once and never touch it again.

Pick 5 · Testing the theory for very little money

8.1/10

Hiccapop Pregnancy Wedge Pillow

A wedge under the bump takes weight off the hip you are lying on, which is a real and often underrated route to the same relief. Two firmness sides let you find out which you prefer without buying twice. It does nothing between the knees, so pair it with a firm cushion and you have a complete setup for a fraction of a U.

Shape
Wedge
Support
Belly, back or under the hip
Bed footprint
Pocket-sized
Firmness
Two-sided, firm and soft

What works

  • Cheapest route to meaningful relief
  • Two firmness sides to compare
  • Takes belly weight off the loaded hip
  • Travels in a suitcase

What it gives up

  • Nothing between the knees — the main fix is missing
  • Small enough to migrate overnight
  • No head or back support
  • You will want a second piece

Skip it ifyou want one purchase to solve the whole problem.

Pick 6 · A favored side and a tighter bed

7.9/10

INSEN C Shaped Pregnancy Pillow with Jersey Cotton Cover

Covers head, belly and knees from one piece on your preferred side, with a cotton cover that is pleasant against skin. For hip pain it is adequate rather than ideal, because fiber fill in the knee section is exactly where compression hurts you. Good general pillow, middling answer to this particular complaint.

Shape
C, one-sided
Cover
Jersey cotton, removable
Support
Head, belly, knees
Bed footprint
Moderate

What works

  • All three support points from one object
  • Jersey cotton cover, removable
  • Half the footprint of a U
  • Reasonable price

What it gives up

  • Fiber knee section compresses, which is the specific failure here
  • One side only
  • No back support
  • Bulky to wash

Skip it ifhip pain is your main reason for buying — spend the difference on firmness instead.

Questions people actually ask

Do pregnancy pillows help with hip pain?

Yes, by keeping your pelvis stacked rather than rotated. Something firm between the knees stops your top leg falling forward, which is what concentrates load on the hip you are lying on.

The qualifier is firmness. A soft pillow between the knees has compressed within a couple of hours and the gap it created has gone.

Why does my hip hurt more at night than during the day?

Because at night you hold one position for hours, loading a single hip continuously, and pregnancy has made the ligaments around the pelvis more relaxed and less tolerant of that.

During the day you shift constantly, which spreads the load. That is also why the pain is often worst in the early hours rather than when you go to bed.

Should I buy a whole pregnancy pillow or just a knee pillow?

If hip pain is your only complaint, a firm knee cushion plus a wedge under the bump costs far less than a full-body pillow and addresses both causes.

A full pillow becomes worth it when you are also dealing with back ache, reflux or constant turning over, because one object handles all of it.

Would a softer mattress help?

On a firm mattress, often yes — the hip needs to be able to sink slightly rather than being held on a hard plane. A topper is the cheap version of that experiment.

On an already soft mattress it can make things worse by letting your pelvis sag out of line, which is the same rotation problem from the other direction.

Will the hip pain go away after birth?

For most people the night-time hip ache from side-sleeping eases quickly once the bump is gone and you can lie how you like again.

Pelvic pain that persists past the early weeks is worth raising with your provider rather than waiting out, because pelvic-floor and pelvic-girdle physiotherapy is effective and the referral takes time.
