The short answer
Sleep, pelvic pain, swelling and heartburn are the four problems, and the one job of this trimester is organizing the after before you need it. Nursing bras around 36 weeks, and the postpartum kit bought and unpacked while you still have the capacity to make decisions. That is the highest-value thing on this page.
What this trimester is like
Weeks 28 to 40, and everything physical gets louder. MedlinePlus's material on aches and pains in pregnancy covers most of it: sleep that will not come, pelvic pain, swelling, heartburn, breathlessness, and the constant need to urinate returning.
It is also the trimester where everything takes longer, which is the practical argument for preparing early. Doing anything at 39 weeks is harder than doing it at 34.
Sleep, which is the main complaint
At full term you need three things at once: the bump supported, the lower back supported, and the knees separated. That is when a full-body U-shape stops being excessive — though it does take over a bed, which is a real consideration if you share one.
If sleep is the thing ruining your days, third trimester insomnia covers more than the pillow. If you wake with heartburn, heartburn at night covers the positional side. And pillows for hip pain covers the specific ache that wakes most people at 3am.
Pelvic pain, and the belt mistake
Pelvic girdle pain peaks in the third trimester, and it is routinely treated with the wrong product.
If the pain is in your lower back, a wide belly-lifting support belt is the right tool.
If the pain is in your pubic bone, groin, or across the back of your pelvis, that is pelvic girdle pain, and the right tool is a narrow sacroiliac belt worn low across the joints rather than a wide belt lifting your bump. NHS guidance on pelvic pain in pregnancy and ACOG's material on pelvic support cover it, and a referral to a physiotherapist is worth asking for.
See SPD relief, belts for pelvic pain, and belt vs pelvic belt.
Swelling: the ordinary kind and the urgent kind
Ordinary swelling builds through the day, affects feet and ankles most, and improves with elevation and compression. NHS guidance on swelling in pregnancy describes it.
Swelling that needs assessing now is sudden, or severe, or in your face and hands, especially with a severe headache or visual changes. That combination can indicate preeclampsia and it is urgent rather than uncomfortable.
Knowing the difference is the single most useful thing on this page. See swollen feet relief.
The one job: prepare for after
This is the trimester where buying ahead genuinely pays, and the list is short.
- Nursing bras, around 36 weeks. Two, stretchy and wireless, not a precise fitting. Sizing at 36 weeks is less wrong than at 20 and still an estimate. See when to buy nursing bras and best nursing bras.
- The postpartum kit. Peri bottle, maternity pads, mesh or disposable underwear. Buy it, open it, and put it in the bathroom you will use. See recovery kits.
- The hospital bag. A gown rather than pajamas, a robe, dark and loose clothes, and the peri bottle with its bag.
- Read the timeline once. Postpartum recovery timeline. Twenty minutes now saves a lot of searching at 3am later.
What is still too early: a postpartum wrap, which goes on after birth and whose timing your own team advises after a caesarean, and a breastfeeding pillow, which can wait.
What to stop buying
Maternity clothes, mostly. At 34 weeks a new pair of maternity jeans gets six weeks of wear, and the money goes further on the postpartum list — where your maternity clothes will still be what you wear anyway. See postpartum comfortable clothes.
How we scored these
Six axes: usefulness this trimester, usefulness after birth, comfort at full term, ease of use when you are large, discretion and value. "Easy to use when large" is here because it is the axis that changes everything at 38 weeks — compression socks are excellent and genuinely hard to put on, and pretending otherwise would be unhelpful.
We read the current listings and the reported experience; we have not used these. The method is on how we pick.

- A reduction or change in your baby's movements — always, at any hour, and never wait until morning
- Sudden or severe swelling of the face or hands, a severe headache, or visual changes — possible preeclampsia
- Vaginal bleeding, or fluid leaking
- Regular tightening or contractions before 37 weeks
- Severe or constant abdominal pain
- Severe one-sided calf pain, warmth or swelling, or chest pain or breathlessness
- Fever, or pain on passing urine
- Severe itching, particularly of your hands and feet
- Pelvic or back pain that stops you walking
- Persistent low mood or anxiety
Every pick, in detail
Scores below are ours. They come from the published specification sheets and from the patterns in what buyers consistently report — not from wearing these ourselves.
Top pick · Full-term sleep, if you have the bed for it
8.8/10Momcozy U Shaped Full Body Maternity Pillow
The point in pregnancy where a full U-shape stops being excessive and starts being the right answer. At full term you need the bump supported, the back supported and the knees separated at the same time, and nothing else does all three without rearranging.
- Shape
- U, wraps both sides
- Support
- Head, belly, back, knees
- Cover
- Removable, machine washable
- Bed footprint
- Wide — plan for a queen or larger
What works
- Supports bump, back and knees simultaneously
- No rearranging when you turn over
- Removable washable cover
- The most support available in one object
What it gives up
- Wide footprint — plan for a queen or larger
- Warm, with a lot of fill against you
- Awkward to move when you are large
- Takes over the bed, which affects a partner
Skip it ifyou have a double bed and a partner — buy a compact pillow instead.

Pick 2 · Pelvic rather than lumbar pain
8.6/10Serola Sacroiliac Belt for SI Joint Dysfunction
The right belt for the pain that peaks now and gets treated with the wrong product. Pelvic girdle pain is not back pain, and a narrow belt across the SI joints below the bump addresses it where a wide belly-lifting belt does not.
- Type
- Narrow sacroiliac (pelvic) belt — not a belly support
- Sits at
- Across the SI joints, below the belly
- Sizing
- By hip measurement, e.g. Large fits 40-45 in
- Design
- Non-slip, clinically oriented
What works
- Correct product for pelvic girdle pain specifically
- Sits below the bump, so it still works at full term
- Sized by hip measurement rather than one-size
- Non-slip, so it holds position while walking
What it gives up
- Wrong product for lumbar back pain
- Does nothing to lift the bump
- Needs the right measurement to work
- Visible under fitted clothes
Skip it ifyour pain is lumbar — buy a belly-lifting support belt instead.

Pick 3 · Swelling at its worst
8.4/10Momcozy Pregnancy Compression Socks 15-20mmHg Wide Calf
Swelling peaks now and this is the intervention that works. The wide-calf cut matters more than it did a trimester ago, because standard socks dig in once calves have changed — and a sock that digs in is a sock you stop wearing.
- Compression
- 15-20 mmHg
- Calf
- Wide-calf cut
- Height
- Knee high
- Toe
- Closed
What works
- Wide-calf cut, which standard socks lack
- Addresses heaviness as well as visible swelling
- Still useful after birth
- Everyday 15-20 mmHg band needs no fitting
What it gives up
- Genuinely hard to put on at full term — ask for help
- Warm in summer, when swelling is worst
- Check firmer compression with your provider
- Few color options
Skip it ifyou have sudden or severe swelling — that needs assessing, not compressing.

| Comfort at full term | 8.0 |

Pick 4 · The buy-ahead purchase, at around 36 weeks
8.8/10Kindred Bravely Essentials Crossover Nursing Bra Wire Free 2 Pack
The one thing worth buying for after, bought now. Around 36 weeks is when nursing bra sizing is least wrong, and a stretchy clip-free crossover tolerates the size change of the first fortnight better than a structured bra fitted precisely.
- Wires
- Wireless
- Access
- Pull-aside crossover, no clips
- Pack
- 2
- Use
- Sleep and lounge
What works
- Right timing: 36 weeks sizes better than 20
- No clips, which is the only access that works half-asleep
- Stretchy enough to tolerate a size change overnight
- Two pack is the right quantity
What it gives up
- Not enough support for a larger bust
- Minimal shaping under fitted clothes
- You will want a structured bra later
- Size is still an estimate
Skip it ifyou are before 34 weeks — it is early to size.

Questions people actually ask
What do I need in the third trimester?
Support for sleep, which at full term means the bump, lower back and knees all at once. The right belt for the pain you actually have. Compression socks for swelling.
And the one job of this trimester: nursing bras around 36 weeks, and the postpartum kit bought and unpacked while you still have the capacity to make decisions.
Why can I not sleep in the third trimester?
Because at full term you need three things at once — the bump supported, the lower back supported and the knees separated — and most setups do one or two.
Heartburn, needing to urinate and breathlessness all add to it. A pillow helps with the position part; the rest needs more than a purchase.
What belt do I need for third trimester pelvic pain?
If the pain is in your pubic bone, groin or across the back of your pelvis, that is pelvic girdle pain and the right tool is a narrow sacroiliac belt worn low across the joints.
If the pain is in your lower back, a wide belly-lifting support belt is the right tool instead. Using the wrong one is the common mistake, and a physiotherapy referral is worth asking for.
When should I buy nursing bras?
Around 36 weeks. Sizing then is less wrong than at 20 weeks and still an estimate, so buy two stretchy wireless ones rather than a precise fitting.
A clip-free crossover tolerates the size change of the first fortnight better than a structured bra.
When should I pack the postpartum kit?
This trimester, and open it rather than leaving it boxed. Peri bottle, maternity pads, and mesh or disposable underwear, put in the bathroom you will actually use.
Everything takes longer at 39 weeks than at 34, which is the whole argument for doing it now.
When is third trimester swelling a problem?
Ordinary swelling builds through the day, affects feet and ankles most, and improves with elevation and compression.
Swelling that is sudden or severe, or in your face and hands — especially with a severe headache or visual changes — needs assessing now. That combination can indicate preeclampsia.
